Disease Management

Chronic conditions account for a large percent of claims dollars as well as expenses related to members being away from the job with chronic illness. MM Solutions provides disease management for the following chronic conditions:

Asthma
Diabetes
Hypertension
Congestive Heart Failure

If other chronic conditions are identified, MM Solutions will implement a program of management to fit the client’s needs.

This program consists of RN educators communicating through mail and by phone with individual members in order to translate information into daily habits of care. As a result of compliance with the new plan of care, the chronic illness may stabilize and thereby save claims dollars and time off from work. This results in a win-win situation for the employer and member as well.
