Case Management (CM)

MM Solutions views case management and catastrophic case management as a vital part of the overall program for our clients. Our philosophy is if a problem is identified early, steps can be taken to prevent worsening of the condition thereby minimizing the severity of the outcomes. For those who are dealing with chronic or debilitating conditions, case management serves as a stabilizing force which enables the member and families to more effectively deal with health issues.

Our case management staff uses state of the art predictive modeling software to identify those cases which hit designated disease and condition targets, which in turn enables MM Solutions to notify the employers of those members who will hit 50% of the stop loss specifications. This also enables earlier intervention in the process by the case manager.

Case management works closely with providers to ensure all concerns are addressed, and all appropriate cost saving measures are employed in the care of our members.
